We inform patients of program benefits, obtain consent forms, and onboard them.
We identify and procure devices and software suitable to practice’s needs.
We program all devices, assign them to patients, and train personnel on usage.
We enter patient demographics, and your preferred vital parameters for each patient, and assign caregivers.
We arrange for, and enforce, vitals capturing for all enrolled patients.
We monitor, manage, and coordinate care per the patient’s needs and applicable billing requirements, as appropriate.
